    There is a demand for advanced practitioners specialising in the case management of multiple long-term conditions in primary care. The course will allow practitioners to develop an integrated and structured approach to the care of those with highly-complex needs while further enhancing their knowledge of research and evidence-based health and social care.

    Designing Research Projects
    This module provides you with the necessary knowledge and skills to design and manage relevant quantitative and qualitative research projects.

    Dissertation
    This module enables you to complete an independent research project. You can discuss areas of interest with an allocated supervisor.

    Evidence-based Practice
    The evidence-based practice module develops skills in studying at master’s level, enabling you to make the transition to postgraduate study. It also introduces you to the key concept of evidence-based practice within a health and social care setting. Throughout the module you will develop practical skills in searching literature, critically evaluating and managing change. You will be able to focus these skills to your own specialist area of practice within the assignment.

    Clinical Assessment & Management of Patients with Long-term Conditions
    This module equips you with the necessary knowledge and skills to clinically manage patients with long-term conditions in primary care. You will develop a robust knowledge based on the best available evidence, combined with a flexible and adaptable approach to the clinical assessment and managing patients with long-term health conditions.

    Lectures and skills workshops develop your knowledge of anatomy, pathology and physiology as applied to the diagnostic process. Facilitated workshops develop your skills in taking clinical history and examinations which are consolidated in clinical practice. Seminars explore how you clinically manage long-term conditions.

    Supportive Care for Patients with Long-term Conditions
    Providing supportive care to patients with multiple long-term conditions is a significant challenge. Faced with multiple diagnoses and complicated treatment regimes, these individuals have to contend with psychological, social, economic and environmental factors that exacerbate their illness. This module will help you respond to these psycho-social needs in an analytical and innovative way.

    Course structure
    Core modules focus upon evidence-based practice and research. Specialist modules address diverse clinical and organisational issues in the management of patients with long-term health conditions. You will also have the opportunity to carry out an independent research study.
