Objectives
You'll develop the cognitive, psychomotor and clinical-reasoning skills essential for a practitioner specialising in the case management of multiple long-term conditions. You'll gain a comprehensive and critical understanding of clinical anatomy and the pathophysiological changes associated with long-term conditions so you can assess and diagnose a new illness or the exacerbation of an existing illness. You'll develop your ability to engage with a broad range of stakeholders to deliver supportive care to this group of patients.
Entry requirements
Applicants should be registered practitioners educated to degree level. Applicants with other qualifications and experience will be considered if they can provide evidence of their ability to study at master's level. All applicants should be employed by a primary care organisation which can provide them with a suitable clinical mentor for the duration of the course and have secured sponsorship from their employing organisation.

Long-term conditions present a significant challenge to practitioners in health and social care.
Course structure
There are two modules, Clinical Assessment and Management of Patients with Long-term Health Conditions and Supportive Care for Patients with Long-term Health Conditions. These modules are complementary to each other and should ideally be undertaken over one academic year.
